Community Health Improvement Plan, 2024

The Sacramento County Community Health Improvement Plan (CHIP)​ is a 5-year action plan aimed to coordinate efforts and target resources to address root causes of health inequities in Sacramento County.  The plan was developed and will be implemented by a CHIP coalition consisting of local organizations, individuals, and County representatives.

SCPH Community Health Assessment, 2023

This Community Health Assessment (CHA) ​ is part of an ongoing community health improvement process and provides data that assists us with identifying priority issues affecting health in Sacramento County. We will use the CHA as the foundation for health planning processes in the future.

Health Officer Order: Health Care Facility Transfers

To reduce the spread of infections, including those caused by multidrug-resistant organisms, the Sacramento County Public Health Officer has issued an Order requiring health care facilities that transfer a patient to another facility to submit a completed Health Care Facility Transfer Form​ to the receiving facility.​​
