Reportable Diseases and Conditions


​​​​​Health care providers are responsible for reporting a case or suspect case of a disease or condition as indicated in the Title 17 ​California Code of Regulations Reportable Diseases and Conditions to the local health department where the patient resides. Refer to the Title 17 Reportable Diseases and Conditions for more information.


Note the following locally reportable diseases and conditions:

Carbapenemase-producing organisms (CPOs) - all organisms

Carbapenem-resistant organisms (CROs) ​- all organisms

Chlamydia

  • Pregnant women
  • Children <13 years old

Influenza (lab-confirmed) cases of ANY AGE, for those associated with:

  • Intensive care unit (ICU) admission
  • Deaths

Respiratory syncytial virus (RSV) for children <5 years old, for those associated with:

  • Intensive care unit (ICU) admission
  • Deaths​

Report diseases and conditions using the appropriate Confidential Morbidity Report (CMR) form or electronically through the California Reportable Disease Information Exchange (CalREDIE) Provider Portal.

Report outbreaks using the Outbreak Reporting Module.
