ABOUT THE PR​OGRAM

California Children's Services (CCS) is a program within Sacramento County's Division of Public Health that provides diagnostic and treatment services, medical case management, and physical and occupational therapy services to children and young adults under 21 years of age with CCS-eligible medical conditions.

​​​​​​​​​​​MEDICAL THERAPY PROGRAM

The California Children's Services (CCS) Medical Therapy Program (MTP) provides occupational and physical therapy services to children birth to 21 years of age who have medically eligible conditions. Therapy is provided without cost to the family, regardless of income, at the Medical Therapy Unit  Learn more about the Medical Therapy Program.


CASE MANAGEMENT PROGRAM

Each CCS client is assigned to a Medical Case Management Nurse (MCMN) and a Health Service Coordinator (HSC).  Family Navigator (FN) is available to all of our CCS clients/families.   ​

Call CCS When:
  • You have questions about your CCS.
  • You just moved to Sacramento County and your child has CCS.
  • You have questions about your Medi-Cal.
  • You can't get in to see a specialist.
  • You have questions about or can't get your therapy, medical equipment, or supplies.
  • You can't get a ride to your doctor's appointments.
  • You need referrals to adult specialists.​

TO QUALIFY

Your child may qualify if he/she meets all four CCS criteria:
  • Age eligible: Your child is under 21 years old.​
  • Medically eligible: Your child has a physical disability or medical condition that is covered by CCS.
  • Residentially eligible: Your child lives with a parent or guardian who lives within Sacramento County.
  • ​​Financially eligible: Your child has Medi-Cal, Optional Targeted Low Income Children's Program (OTLICP), your family’s adjusted gross income is less than $40,000/year, or out-of-pocket medical expenses are expected to be more than 20 percent of your family's adjusted gross income.

HOW TO REFER A CHILD TO CCS​

Anyone can refer a child to CCS, including physicians and families. 
Families can:​
  • Complete the CCS application, See how to submit the application below.
  • Include all of the information requested.
  • ​The application will be reviewed, and the family will be notified of the next steps in the process.
  • Apply to Medi-Cal if CCS asks you to do so.
