Health Center patients can access personal health information online through MyChart.

Medical Records Requests:​
To request copies of your medical records, please submit an originally signed Authorization to Release Medical Records (form 2099​​) (electronic signatures can not be accepted) ​and a copy of the patient’s photo ID. 

Requests submitted by a third party must also include a check or money order for $15 to cover processing.

Submissions can be emailed to DHS-FAX-PHSMRRequest@SacCounty.gov or faxed to 916-854-9251.  Requests are processed in the order received and should be sent within 10 days from the date the request is received.​

For additional information or to check on your request after 10 days, you may call 916-874-9298.  In order to help us respond to phone calls in a timely manner, please only leave one voicemail message.​



COVID-19 (2019 Novel Coronavirus)

Sacramento County Health Center is currently providing COVID-19 vaccines​ to all patients six months​ of age and older, including COVID-19 booster vaccination to qualifying individuals.​​
