Health Care for the Homeless

The County of Sacramento Department of Health/ Primary Health Services (DHS/PHS) division is a recipient of a HRSA 330h Federal grant also known as the Health Care for the Homeless (HCH) grant. The Health Care for the Homeless team of physician lead medical and behavioral health professionals provide medical and enabling services for homeless individuals in Sacramento County. The staff provide care at the HCH satellite clinic, located at Loaves and Fishes, the Mobile medical van and Street Medicine.

The satellite clinic is located within the Loaves and Fishes service area, in the Delaney Center, and is operated each day Monday – Friday 730-1130am. Services include primary and public health care and behavioral health visits (on Tuesdays).  The staff also provide tuberculosis testing/screening. Medications are available based on insurance type. Transportation vouchers are given to get to labs and radiology services at Sacramento County Health Center located at 4600 Broadway.

The Mobile Van and Street medicine physicians provide health care services in various remote locations throughout Sacramento County where homeless camps are prevalent. The Mobile clinic provides physician evaluations of acute as well as chronic health care problems and health education. In addition, Public Health staff conduct teams of Communicable Disease Investigators for Sexually Transmitted Infection (STI) screenings and Public Health Nurse Outreach collaboration activity with the Mobile medical van and Street Medicine physicians. Street Medicine is provided in the camps and along the streets, parks and Sacramento River. Patients are invited to van for privacy if requested.

The line graph above provide a 5 year snapshot of unique homeless encounters by DHS/PHS Homeless and Clinic staff. The Homeless had declining numbers in 2018 due to facility challenges. However since 2020, the clinic has resumed and increased services including the Mobile medical van and Street medicine.

Key partnerships that enhance these services include Loaves and Fishes, University of California at Davis School of Medicine, Sacramento Covered, Sacramento County Public Health and other interagency divisions.
